Hartlepool, Swansea, Rotherham and Leeds all progressed against opponents from a higher division
League One side Hartlepool produced a stunning comeback to defeat Premier League side West Brom after extra-time.
The home side looked to be heading for a famous victory through Joel Porter's 61st minute opener, but Robert Koren equalised for the Baggies with just three minutes remaining.
Hartlepool fought back, however, and extra-time goals from David Foley and Richard Barker gave them a famous victory.
In a night of lower league triumph, Swansea beat Premier League newboys Hull City 2-1 after extra-time.
Dean Windass opened the scoring for Hull, but goals from Gorka Pintado and a Jordi Gomez penalty were enough to secure victory for the Swans.
Stoke edged through to the third round with a narrow 3-2 win away at Cheltenham.
Leeds produced a wonderful performance to beat Crystal Palace 4-0, while Argentinean striker Emmanuel Ledesma scored a hat-trick for QPR as they beat Carlisle 4-0.
Wigan avoided a potential banana skin by beating Notts County 4-0, with Henri Camara scoring twice, while Southampton beat Birmingham 2-0 at St Mary's.
Ipswich beat local rivals Colchester 2-1, while Middlebrough made light work of Yeovil, winning 5-1 at the Riverside.
League Two side Rotherham beat Championship outfit Wolves on penalties, while
Reading beat Luton Town 5-1.
Derby put their miserable form behind them to record an impressive 1-0 away at Preston.
