West Ham needed extra-time to see off a spirited Millwall in a match that will be remembered more for crowd trouble than the football.
Burnley, Hull and Bolton all also moved into the next round of the Carling Cup.
League One side Millwall took the lead in the first half through Neil Harris and looked as though they had done enough before Junior Stanislas equalised with just three minutes remaining.
Stanislas added another from the penalty spot in extra-time and Zavon Hines sealed the win for the Premier League side.
But it was the supporters who will make the headlines after numerous pre-match clashes between fans added extra spice to the encounter.
There was huge security presence inside Upton Park but this did not stop Hammers fans from invading the pitch when each goal went in.
The Lions shocked the home side after 26 minutes when Harris volleyed in from close range following a poor header from West Ham defender Danny Gabbidon.
And it wasn't until the 88th minute that West Ham drew level when Stanislas scored to take the tie into extra-time.
A Millwall hand ball gave Stanislas the chance to give West Ham the lead from twelve yards, with the youngster making no mistake with the spot-kick.
Just two minutes later, fellow academy product Zavon Hines fired in another after a good run and in so doing booked West Ham's place in the next round.
Elsewhere in the competition, Premier League new boys Burnley also left it late as they defeated League One Hartlepool 2-1 after extra-time thanks to a brace from Steven Fletcher.
Hull had no such problems as they brushed aside Southend United 3-1. Bolton were also in action, winning 1-0 away at John Barnes' Tranmere Rovers.
