Premier League Sunderland were sent crashing out of the Carling Cup at the first hurdle after League One side Luton Town inflicted a 3-0 upset at Kenilworth Road.
The Black Cats finished a man down after Greg Halford picked up a second booking for a deliberate handball but they already looked a beaten side before the dismissal.
David Bell's ferocious free-kick brought the home crowd to their feet after 13 minutes before Paul Furlong looped a finish over Darren Ward to double the advantage.
The former Chelsea forward further blackened Roy Keane's mood with a goal on 75 minutes to complete a memorable win.
Elsewhere Hull claimed another Premier League scalp with a 1-0 win over Wigan at the JJB Stadium.
The Tigers had Stuart Elliot to thank after his fizzing 20-yard volley crashed past Mike Pollitt just after the half-hour mark.
Chris Hutchings' side went close through Michael Brown and Caleb Folan but they will now have to concentrate their efforts on just two fronts.
Derby have also had their fixture list lightened after they lost to Championship side Blackpool on penalties at Pride Park.
Mo Camara put the Rams in front, but an 86th-minute goal from Latvian defender Kaspars Gorkss meant the game rolled on into extra time.
Craig Fagan appeared to have settled the tie with a second for the home side eleven minutes into the additional period, but Gorkss struck in the last minute to take it to spot-kicks.
