Cristiano Ronaldo scored on his first start of the season as Manchester United beat Middlesbrough 3-1 at Old Trafford in the Carling Cup.
Ronaldo headed home a Ryan Giggs corner after 25 minutes to give United the lead, but Adam Johnson equalised with a brilliant 30-yard volley after 56 minutes.
Giggs got the winner with 11 minutes remaining and Nani added an injury-time third, but the night was marred by a horrific tackle by Middlesbrough captain Emanuel Pogatetz on Rodrigo Possebon.
Pogatetz received a straight red card and play was delayed for several minutes while Possebon was stretchered from the field.
Burnley produced an upset to beat Fulham at Turf Moor and progress to the fourth round.
Substitute Chris Eagles set up another sub Jay Rodriguez to score the late winner.
Gianfranco Zola suffered his first defeat as West Ham manager as his side lost 1-0 away at Watford. Hayden Mullins' 70th minute own goal was the difference between the sides in the London derby.
Liverpool made hard work of beating Crewe 2-1 at Anfield. Daniel Agger opened the scoring in the 15th minute, but Michael O'Connor equalised 10 minutes later. Brazilian international Lucas got the winner 13 minutes into the second-half.
Swansea won the Welsh derby against ten-man Cardiff 1-0 with a 57th minute goal from Spaniard Jordi Gomez.
Leeds twice came from behind to beat Hartlepool 3-2 at Elland Road with Andy Robinson scoring a 90th minute winner.
Rotherham produced an upset to beat Championship side Southampton 3-1.
