Keith Richards was said to be in a serious condition last night after undergoing a second brain operation.
The Rolling Stones guitarist required surgery to remove a section of fractured bone from his skull.
His family kept a bedside vigil at the hospital in Auckland, New Zealand, and a source has revealed all are extremely concerned.
"Everyone is worried sick. This is serious," a source told the Sun newspaper.
Richards, 62, suffered the injury after falling from a palm tree while holidaying in Fiji.
He was flown to Auckland's Ascot hospital after the fall but was initially discharged after a scan.
However, he was subsequently readmitted as his condition deteriorated.
The Rolling Stones are due to commence the European leg of their world tour in Barcelona later this month.
