An investigation has been launched after a senior surveillance officer admitted changing evidence during the inquest into the shooting of Jean Charles de Menezes.
The Independent Police Complaints Commission (IPCC) confirmed that an inquiry had been launched.
A statement released today said: "The IPCC has received a referral from the Metropolitan police service in relation to a matter arising from the inquest into the death of Jean Charles de Menezes.
"This matter will be subject to an independent investigation and we are not in a position to comment further at this stage."
Mr de Menezes, 27, was shot dead by armed Metropolitan police officers in July 2005 after being mistaken for a suicide bomber at Stockwell tube station.
An inquest into his death is currently taking place at Oval cricket ground.
A Met statement added this afternoon: "After appropriate consideration, once we were notified of an issue relating to a witness, an officer based in Specialist Operations, the Metropolitan police service voluntarily referred this matter to the IPCC on October 9th."
