Ten-man Darlington led three times before losing on penalties to Reading in the Carling Cup second round.
The League Two side led through Simon Johnson's 19th minute penalty and a Julian Joachim double, only for Leroy Lita's brace and Peter Mate's equaliser four minutes from the end of normal time to restore parity.
However, Darren Holloway and Neil Wainwright missed for Darlington and Reading won 4-2 on penalties.
Watford almost became Accrington Stanley's second major scalp of the competition before succumbing on penalties.
Stanley, who knocked out Nottingham Forest in the last round, held the Premiership side 0-0 after 120 minutes but Richard Lee saved Paul Mullin's spot-kick and Anthony McNamee became the hero for Watford as they won 6-5 on penalties.
Portsmouth continued their good start to the season by avoiding a potential banana skin away to Mansfield Town with a 2-1 win.
Manuel Fernandes gave Pompey the lead after five minutes before Matt Taylor extended the lead on 33 minutes.
Mansfield pushed for a way back into the game and were rewarded with a goal nine minutes from time from Danny Reet.
Charlton recorded only their second win of the season as they beat Carlisle United 1-0 at the Valley.
The Londoners have a penchant for cup upsets but a goal from Darren Bent early in the second-half sealed their progression.
Bolton needed two goals in the last three minutes from Ivan Campo and Nicolas Anelka to see off Walsall at the Bescot Stadium.
Kevin Nolan had opened the scoring for Bolton on 68 minutes before Martin Butler equalised six minutes later.
However, Campo's 88th minute effort and Anelka's first goal for the club put the Premiership side through.
